As a Director of Nursing (DON) in a skilled nursing facility, survey readiness is always on your radar.
But what if your policies and procedures could be your secret weapon—not just for compliance,
but for confidence.
Here’s how to turn your P&Ps into a powerful tool for survey success:
1. Know Your Policies—Don’t Just File Them
Policies and procedures are more than binders on a shelf. They’re your road map to regulatory compliance.
Action Steps:
> Review high-risk policies quarterly (e.g., infection control, medication administration, abuse
prevention).
> Ensure policies reflect current CMS regulations and state-specific requirements.
> Involve department heads in policy reviews to promote ownership and understanding.
2. Train Staff to Live the Policies
Surveyors don’t just want to see your policies—they want to see them in action.
Action Steps:
> Integrate policies into onboarding and annual competencies.
> Use real-life scenarios to train staff on how to apply procedures.
> Conduct mock surveys and ask staff to explain key policies in their own words.
3. Align Documentation with Policy
If your documentation doesn’t match your policy, it’s a red flag.
Action Steps:
> Audit clinical records to ensure documentation supports your policies (e.g., fall risk assessments,
care plans).
> Use checklists and templates that mirror policy language.
> Provide feedback and retraining when inconsistencies are found.
4. Prepare for Surveyor Questions
Surveyors often ask, “Show me your policy on…” Be ready.
Action Steps:
> Create a Survey Readiness Binder with frequently requested policies.
> Assign a “policy champion” on each shift who knows where to find key documents.
> Practice mock interviews with staff using policy-based questions.
5. Use Policies to Drive Quality Improvement
Policies aren’t static, they should evolve with your facility’s needs.
Action Steps:
> Track incidents, audits, and survey citations to identify policy gaps.
> Update procedures based on QAPI findings and staff feedback.
> Celebrate improvements that stem from policy changes—it builds buy-in!
A successful survey isn’t about perfection—it’s about preparation, consistency, and accountability. When your policies and procedures are understood, followed, and documented, you’re not just survey-ready, you are survey-strong.
